I understand that dogs participating in daycare engage in normal dog-to-dog interactions, play, movement, and social activities.

I acknowledge that even in a supervised environment, risks including scratches, minor injuries, illness exposure, stress, or other unforeseen incidents may occur.

I certify that all behavioral and medical information provided is truthful and complete.

I agree to immediately inform Surefire K9 of any behavioral changes, aggression concerns, injuries, illnesses, or changes in vaccination status.

I understand that dogs displaying unsafe, disruptive, or excessively stressful behavior may be separated from group activities or dismissed from the program.

I understand that admission into daycare is contingent upon a successful evaluation period and that acceptance is at the sole discretion of Surefire K9.

I understand that photos and videos may be taken of my dog during their time at Surefire K9 and may be used for social media purposes.

I authorize Surefire K9 to seek veterinary treatment if my dog becomes ill, injured, or experiences a medical emergency during their stay and I cannot be reached in a timely manner. I accept full financial responsibility for all veterinary expenses incurred. (Required)

Boarding Agreement & Liability Waiver

I understand that boarding dogs involves normal risks, including but not limited to injury, illness, stress, escape attempts, exposure to contagious disease, and behavior changes related to being away from home.

I certify that I have fully disclosed my dog's medical history, behavioral history, bite history, anxiety, reactivity, resource guarding, escape attempts, and any other concerns that may affect their care.

I understand that Surefire K9 will make every reasonable effort to provide safe, attentive care, but cannot guarantee that accidents, injuries, illness, stress-related behaviors, or unforeseen incidents will not occur.

I agree that Surefire K9 may separate, restrict activity, modify handling, or decline future boarding if my dog displays unsafe, destructive, overly stressful, or concerning behavior.

I understand that my dog must be free from contagious illness and external parasites upon arrival and agree to notify Surefire K9 of any known exposure prior to boarding.

I accept full financial responsibility for all veterinary expenses incurred on behalf of my dog during boarding, including emergency treatment.

I understand that boarding reservations are made specifically for my dog and that late pickups may result in additional boarding fees.

If my dog is not picked up within the agreed-upon timeframe and I cannot be contacted, additional boarding fees will continue to accrue until arrangements are made.

I certify that all information provided is accurate and complete. Failure to disclose relevant medical or behavioral information may result in dismissal from future services.
